What are Tear Trough Fillers?

Tear trough fillers are a highly specialized non-surgical treatment designed to address hollows, dark circles, and tired-looking eyes. The "tear trough" is the depression that runs from the inner corner of the eye down toward the cheek. As we age, we lose volume in the mid-face and the skin under the eyes thins, making this hollow more pronounced and creating a shadow that looks like a dark circle.

How the Treatment Works

The procedure involves injecting a specialized, very lightweight hyaluronic acid (HA) filler directly into the hollow area beneath the eye. By restoring volume to this depression, the shadow is eliminated, the transition between the lower eyelid and the cheek is smoothed, and the under-eye area reflects light better, significantly reducing the appearance of dark circles and fatigue.

Are You a Good Candidate?

Tear trough filler is not suitable for everyone with under-eye concerns. It is crucial to have a proper clinical assessment.

  • Good Candidates: Individuals with genuine anatomical hollowing (a physical depression) under the eyes, good skin elasticity, and tight lower eyelids.
  • Poor Candidates: Individuals with under-eye "bags" (fat prolapse), severe skin laxity, or those whose dark circles are caused purely by pigmentation (dark skin tone) rather than shadowing. In these cases, filler can actually make the eyes look puffier or worse.

What to Expect During Treatment

Due to the delicate nature of the under-eye area, this treatment is usually performed using a micro-cannula (a blunt-tipped tube) rather than a sharp needle. A single entry point is made on the cheek, and the cannula is threaded up to the tear trough. This method drastically reduces the risk of bruising, swelling, and vascular complications.

The procedure takes about 20 to 30 minutes. The filler used (such as Teosyal Redensity II or Restylane Eyelight) is specifically formulated for this delicate area and contains lidocaine for comfort.

Results and Downtime

Results are visible immediately, though the area may be slightly swollen. The filler will settle and integrate with the tissue over the next 2 to 4 weeks. Because the under-eye area has very little muscle movement, tear trough fillers last exceptionally long—typically 12 to 18 months, and sometimes up to several years.

Clinical Note: The tear trough is one of the most anatomically complex and high-risk areas of the face to inject, carrying risks of prolonged swelling (malar edema), the Tyndall effect (a bluish tint if filler is placed too superficially), and vascular occlusion. It should only be performed by an advanced medical practitioner (Doctor, Dentist, or Prescribing Nurse) with extensive experience in peri-orbital anatomy.
